At Community First, we believe that good health and wellbeing are about more than medicine — they’re about feeling connected, supported, and confident in everyday life.
Social Prescribing is a person-centred approach that helps individuals take control of their wellbeing by linking them to local activities, groups, and services that meet their practical, social, and emotional needs.
Through Social Prescribing, people can access tailored support to improve areas of life that may be impacting their health — from managing stress and loneliness to finding new ways to stay active, volunteer, or build confidence. Our Link Workers take the time to understand what matters to you, working together to create simple, achievable goals that help you move forward.
By connecting people with their communities and supporting them to make meaningful changes, Social Prescribing helps individuals feel more empowered, resilient, and in control of their own health and happiness.

OUR SERVICE.
Community First delivers the Social Prescribing service for patients registered at North Shore and Glenroyd Surgeries. We accept referrals for anyone within these areas who may benefit from additional support to improve their wellbeing.
Our team can help with issues such as social isolation, low-level mental health concerns, long-term conditions, housing or money worries, lifestyle changes, and connecting with local activities or volunteering opportunities.
Working in partnership with local health, social care, and community organisations, we provide up to 12 weeks of personalised support designed around each person’s individual goals — empowering them to build confidence, strengthen connections, and improve quality of life.

To ensure everyone receives the right support at the right time, our Social Prescribing service is built around four core pillars of care and connection.
PERSONALISED SUPPORT
We focus on you — taking time to understand what matters most and developing a tailored plan to meet your goals and priorities.
CONFIDENCE BUILDING
Our Link Workers provide encouragement and practical support to help you build confidence, resilience, and independence.
CONNECTION & COMMUNITY
We help link you to local groups, services, and activities that support social connection, learning, and wellbeing.
COLLABORATION
We work closely with GP surgeries, local agencies, and community partners to ensure joined-up, effective support for every person referred.

01.
REFERRAL
Referrals can be made via North Shore or Glenroyd Surgeries, through any Community First project, or by self-referral via the contact form below.
Once received, our team aims to make contact within seven working days.
02.
INITIAL CONTACT
Your referral is processed and allocated to a Link Worker who will arrange an initial face-to-face assessment — at our office, your GP surgery, a community venue, or at home if you are homebound.
03.
SUPPORT SERVICES
We provide up to 12 weeks of tailored support. Together, we’ll set wellbeing goals, create a personal plan, and connect you to services that best match your individual needs.
04.
SERVICE
COMPLETION
At the end of your support, a final review is completed to discuss progress and outcomes. With your agreement, a discharge summary is shared with the relevant Blackpool North PCN.

Many things affect our health and wellbeing — from loneliness and stress to money, housing, or long-term conditions. Social Prescribing offers a different kind of help. It begins with a simple conversation, often with your GP or practice team, who can refer you to a Social Prescribing Link Worker.
Your Link Worker will take time to listen and connect you with local activities, groups, and services that can help you feel better and more in control of your health.
It’s about finding the right support for you — whatever your age, background, or circumstances.
Participation is FREE, and no prior commitment is required —
just a willingness to engage in a safe, supportive, and inclusive space.
